Thank you for considering being a chaperone at the 2019 Convention! Please follow these instructions to apply.

1. Complete the Nurse’s online registration form.
2. You will receive a confirmation email with further instructions.
3. Open the Medical Information form.
   1. To complete the form in Google Docs, sign into your Google account, open the form and click File > Make a Copy.
   2. To complete the form on your computer or on printed paper, open the form and click File > Download as, and choose Microsoft Word or PDF.
4. Send your completed Medical Information form using the instructions in the email we send after you register.
5. Read the Chaperone Guidelines.
